Preparing for the Evaluation

  • Providing information: You will be asked to complete specific intake forms prior to the initial meeting.  You will be able to access these forms through this website using a password which will be provided by Dr. Jaret. In addition, you are encouraged to provide Dr. Jaret with any previous evaluations, medical reports, samples of schoolwork or school records which may assist in the development of the evaluation and eliminate the need to duplicate tests previously conducted.

  • Preparing a child for the evaluation: What you tell your child about this evaluation depends on his or her age and ability to understand.  Keep the explanation simple and relate it to your child's understanding of a specific problem such as reading difficulties, difficulties in following instructions, or difficulties in making new friends. Tell your child that you want to better understand how he or she learns so that you can help to make school more fun and less frustrating for him or her.  Explain that by understanding their strengths and challenges you will be able to help them to be more successful socially, emotionally, and academically.

  • The day of the evaluation: The child or adult client should dress comfortably for the evaluation. They should have a proper meal and a good night's sleep before the session. If the client wears eye glasses or a hearing aid, please make sure to bring them with you to the evaluation. If the client takes stimulant medication (such as Ritalin or Concerta), please discuss this with Dr. Jaret in advance of the appointment in order to determine the best time to administer the medication on the evaluation day.
